PACs Proforma example

Ensure you cover areas such as:

Systems review:

Cardiac: Hypertension, Hypercholesterolaemia
No IHD, no chest pain, no palpitations/ irregular heart beat, no SOB on laying flat, no PND, no SOB, no previous cardiac Ix
Resp: No asthma, COPD, no chronic cough, SOB, not on oxygen therapy
GIT: no melaena, haemetemesis, GORD, no liver disease
GUT: No kidney stones, no kidney disease
Endo: no diabetes or last check of BSL 3 months ago- borderline diabetes,
Neuro: No stroke, TIA, epilepsy
Bleeding diathesis: No anticoagulants, no anti-platelet therapies (Nil Aspirin, Clopidogrel, Plavix, Warfarin, Dabigatran)
Previous DVT/PE
Rheum: rheumatoid arthritis, OA in neck: No
Able to walk >2 flights of stairs: Yes
Previous reaction to GA: No
Fevers/chills/shakes/systemic Sx: No
Sees GP regularly

Examination

Mobilises independently
BMI (average body habitus (mild central obesity))
Mouth: healthy mucosa, own dentition (dental review pre surgery- tell liaison nurse they need to see their dentist), able to open mouth fully, Malampatti ?, good jaw protrusion
FROM at neck, no restriction, thyromental distance >6cm?
CV: HS dual nil added, no murmurs, chest clear, JVPNE
no carotid bruits
Or if a paper PAC (draw the lungs symbol with clear arrow across and just add HS dual nil added)
Left hip: no scars, wounds or erythema
Legs: nil swelling, calves soft
Feet: no fungal skin infections or signs of nail bed infections, no ulcers on feet
